Njideka J. Ejinaka, DNP, M.B.B.S, ARNP, PMHNP-BC

Nurse Practitioner

Dr Njideka Ejinaka immigrated to the United States about 21 years ago and currently resides in Trotwood, a suburb of Dayton Ohio. Her educational background in healthcare is well grounded and includes graduating from the University of Nigeria School of Medicine in November of 1998 with an MBBS degree.


Njideka’s quest to further her career and her passion for patient care   motivated her relocation to Florida in 2016. There, she joined the accelerated program for foreign educated physicians, the (FEP) nursing program at Florida International University (FIU), in Miami Florida. She then completed her Master of Science in Nursing studies with a subspecialty in psychiatry (mental health) in July of 2019.

Dr Ejinaka is board certified in Psychiatric -Mental Health by the American Nurses Credentialing Center (ANCC). She has worked at a Geropsychiatric Hospital in Cincinnati Ohio providing care to patients. Furthermore, Njideka has provided nursing services including psychiatric mental healthcare to residents living in various nursing homes in and around Dayton Ohio. In August of 2022, Njideka completed and earned a Doctor of Nursing Practice (DNP) degree from Wilmington University, state of Delaware.

Dr Ejinaka is married with 3 children. She enjoys spending time with her family, watching African especially Nigerian home movies, and cooking her native dishes.

Dr. Samantha Yates

PharmD, BC-ADM

Samantha Yates serves as the Clinical Pharmacist at Omni Wellness Group.


She received her PharmD from Cedarville University in 2018. Following graduation, she completed a one-year Post-Graduate Year 1 (PGY-1) Ambulatory Care residency at Western Medicine Family Physicians in Fairborn, Ohio, where she assisted in chronic disease management (diabetes, high blood pressure, asthma, COPD, and high cholesterol, Medicare annual wellness visits, transitions of care, and chronic care management. Following residency, she became board certified in advanced diabetes management. She is excited to join our team here at Omni Wellness Group to provide collaborative care for disease management for patients.

Samantha met her husband, Seth in high school and they married in 2015 and they have two pups, Zoey and Izzy. In her spare time, she likes to read, watch movies, exercise, and play board games with her family.

Jessica Schloss, MA

Office Manager

Jessica Schloss is an experienced certified medical assistant that also serves as the practice office manager.


Jessica was born and raised in Johnson City, Tennessee and made her move to Troy, Ohio in 2016.

She graduated from East Tennessee State University with an associates degree in Allied Health and completed her certification of medical assisting through the National Healthcare Association. She has past experience in many fields including chiropractic, orthopedic, dermatology, pulmonary and internal medicine.

Leslie Stegall


Leslie Stegall graduated from The Ohio University with a master of science in nursing, specializing as a family nurse practitioner.


She has been in nursing for 14 years, the last four of which have been spent as a family nurse practitioner in rural Ohio. After being frustrated with the volume of patients who were presenting with uncontrolled mental health disorders and substance use disorders, Stegall decided to pursue a post-master’s psychiatric mental health nurse practitioner certificate from the University of Cincinnati. She felt driven to pursue this specialization in order to become part of the solution instead of accepting the fact that reliable mental health resources in the area were scarce. Psychiatry has become a passion Stegall has embraced, and she will graduate with a PMHNP degree in December 2021. Stegall enjoys spending time listening to her patients and helping them get to the root of their issues. She goes into each appointment with an open mind and allows the patient to guide her through their perspective. This allows her to create a treatment plan with her patients, not for her patients. She looks forward to partnering with the patients at Omni Pacific and providing exceptional mental health care.

Cindy Fleming MSN, FNP-BC

Nurse Practitioner

As a graduate from Chamberlain University, Cindy received her master’s degree in the science of nursing specializing as a family nurse practitioner.


While she is a dedicated family practicing provider, she also provides care through a GI clinic as a gastroenterologist specialist in Lima, Ohio, and previously as an in-home practitioner for Humana Insurance Group. Starting her medical career over 22 years ago in bedside nursing with a degree from Edison State College, Cindy’s experience spans the surgical, physical rehabilitation, and telemetry units, and three years as a dedicated acute and chronic dialysis provider managing anemia and other treatments both in and out of the hospital. Having spent 2 years in quality improvement where she worked alongside acute care providers and hospitalists studying patient cases, to improve patient outcomes and instill patient safety measures and protocol, she has developed insight as to the value of quality patient care. Cindy’s goal has and continues to be, to provide the best care, advice, and guidance she can for her patients using her knowledge, skills, and experience to offer excellent multifaceted care in a holistic, evidence-based and professional manner. Outside of working at OMNI Wellness Group, Cindy is married and has 4 teenage children who keep her busy and young, 4 standard poodles, a cat, a bunny, and chickens on a small farm where she and her husband voluntarily host an annual camp-out for youth ministry.

Nicole Partin

Office Receptionist & Medical Assistant

Nicole joins the Omni Wellness Group (OWG) with over 10 years of progressive Certified Medical Assistant (CMA) experience in both large and small primary care practices.

She recently served as a medical assistant for a local primary care practice and a regional specialty clinic. A graduate of Ivy Tech Community College, Nicole brings a patient-centered approach to health and wellness that creates office efficiency and a supportive environment. She lives in the Troy area.

Bruce Clapp

Managing Director

Bruce leads the OmniWellness Group management team and is directly responsible for strategic direction, creating efficient operational support, and identifying new opportunities for patient service and practice growth.

Adam Middleton

Clinic Operations Director

Adam is directly responsible for the daily OWG clinic management to bring world-class patient care to the community.


Adam Middleton has committed his entire professional career to healthcare.  Adam works as a business consultant for hospitals, physician groups and healthcare related entities. Since 2008, Adam has consulted with physician practices, post-acute providers and hospitals with particular focus on physician compensation, fair market value, management, operations, finance and strategic planning. Among other projects, his key consultancy roles included the merger of two hospital managed service organizations, the start-up of physician practices, and the negotiation of numerous physician-hospital acquisitions.  He also leads strategic planning for physician groups and hospital service lines.

He founded Healthcare Advisory Network in 2011 to specialize in profitable growth strategies for hospitals, physicians and healthcare companies. HAN affiliates with subject matter experts and numerous organizations to assist clients find the best solutions for issues.

Prior to consulting, Adam was an executive at two major healthcare organizations in the greater Dayton area; Dayton Heart Hospital (physician owned jointly with MedCath) and Premier Health Partners.

As a lifelong Ohioan, Adam has been actively engaged in serving his community where he lives and raises his family. He has served on the boards of directors of The Far Hills Surgical Center, The Dialysis Centers of Dayton and as board chair of Life Essentials.

Adam is current member of the National Society of Certified Healthcare Business Consultants and the MGMA.  He has been a member of the Ohio Hospital Association Legislative Committee, The Greater Dayton Area Hospital Association Disaster Committee, the Dayton Regional EMS Council and the Dayton Area Chamber of Commerce as an ambassador and a Legislative Committee member.

Adam served the community of Bellbrook, OH for 13 years as a first responder, both as a volunteer firefighter and an EMT.

Sheila Petrequin

Coding, Billing and Collections

Sheila is directly responsible for managing the OWG patient billing, insurance provider relations and revenue collections.


Sheila Petrequin has been employed in the world of Revenue Cycle Management (RCM) for her entire career. She attended high school and college in southern Ohio and her career began as a supervisor at Springboro Clinic in Springboro, Ohio.

For 13 years Sheila worked as a manager for a local RCM and then decided to further enhance her career by going to work as a trainer/consultant for medical practices that were implementing Physician’s Accounts Receivable computer software. She traveled from coast to coast for five years installing, implementing, consulting and training employees on the nuances of their new software. This was the early stages of converting from a paper environment to one of computers.

After the additional experience “in the field”, she became employed as the Chief Operating Officer of a 400 physician and 150 employee medical practice. After a few years Sheila founded Maxx Medical Inc.

For the last 15 years Sheila has owned her own medical billing and revenue cycle management company, Maxx Medical, Inc., featuring “maximum results in minimum time”. Sheila is one of only six CHBME (Certified Healthcare Billing Manager Executive) associates in the state of Ohio. This certification is a distinction through HBMA an organization of over 1500 members designed specifically for RCM companies.  She has a diverse background in optimizing reimbursements through a thorough knowledge of coding for specialty practice operations. Her experience in Radiation Oncology, Physiatry, Podiatry and Pain Management billing is unequalled. She also has extensive knowledge in fee schedule management, E&M chart auditing, healthcare credentialing and contract negotiation.

Sheila serves her community and those around her in various volunteer roles. Her motto is GIVE BACK.


Healthcare Advisory Network

Healthcare Advisory Network (HAN) is charged with the operational management of the Omni Wellness Group clinic.

In partnership with the Omni Wellness Group, HAN is leading the development of the clinic process, staff management, employer healthcare connections and telemedicine connections to specialists in all manner of care for our patients.

Founded in 2011 to focus on hospital consulting, HAN now works with complex clients in large healthcare systems to single physician practices and related health care entities. HAN brings scalable practice management expertise to the growing Omni Wellness Group clinic network.

Kenneth A. Mankowski, D.O., Board-certified Neurologist

Kenneth A. Mankowski, D.O., is a board-certified neurologist specializing in the diagnosis and treatment of multiple sclerosis (MS) and general neurological conditions. Dr. Kenneth Mankowski offers the diagnosis and treatment of conditions that affect the brain, spinal cord and nerves throughout the body. He has a special interest in working with patients with multiple sclerosis (MS).

Dr. Kenneth Mankowski received his Doctor of Osteopathic Medicine degree in 1995 from The University of Health Sciences College of Osteopathic Medicine in Kansas City, Missouri.

Dr. Mankowski completed an internal medicine internship and a one year emergency medicine residency at Mount Clemens General Hospital in Mount Clemens, Michigan. He subsequently completed a residency in neurology at Botsford General Hospital (affiliated with Michigan State University) in Farmington Hills, Michigan, He also completed a fellowship in neurophysiology and neuromuscular disorders with a focus on electromyography (EMG) and electroencephalography (EEG) testing at Henry Ford Hospital in Detroit, Michigan.

Dr. David Lee

Dr. Lee is happily married with three children, a son-in-law, and two grandchildren. He received his first doctorate degree in 1983. Second Doctorate in Public Health, 1998. Still in private practice today, he has participated and collected extensive credentials.
